If you're referring to thin clients such as those offered by HP, the big difference is manageability and they are fit-for-purpose. With those devices you can easily:
1. Configure them so they forget any changes made to the local machine when rebooted. As the hard drive forgets everything during the session you don't need to worry about config changes, software accidentally installed etc.
2. Lock them down so all they can do is run the thin client software.
They also typically run a cut-down version of MS Windows also designed for this purpose and so require little to no maintenance such as Windows Updates etc.
