As exICL (retired 30+ years ago) who had nothing to do with Horizon but is belatedly trying to follow the inquiry, I have questions regarding the relationship between PO FJ and the spms.
Were the essential processes needed by PO to support in place, with resourcing at the level needed by the SPMs from day 1.
Was the PO structure modified to cope with an online system where all units had to be aware of issues happening all over the country
If senior management of PO were not aware of their staff prosecuting SPMs, how could they ensure that these staff were working to standards set by the PO.
Did FJ understand the legal requirements of Expert Witnesses as apart from Technical experts.
Did FJ compromise their deliverables in order to meet timescales driven by politicians (and or Royal Mail Group) looking for quick results
Etcetera
I know nothing of Agile but software projects have deliverables which are time related, needing to be accurate from day 1, have to be fully defined to produce acceptable results. I would argue that Horizon has to be correct in order to allow the production of acceptable auditable annual accounts on time to meet financial timescales. This is not imho compatible with Agile with it's try, throw away, and try again attitude
