At least powering several racks from a single UPS was intentional, if unwise.

At a client site there were the usual two UPS's and racks had dual PDUs so you could power equipment with more than one power-supply from different protected circuits. All fine and dandy. Until one day some maintenance was required on the main panel, which meant powering down one of the feeds. This should not cause a problem, everything was redundantly powered, right?

Electrician trips the breaker and a row of racks powers down.

It turned out that at the data-centre build someone had accidentally wired both the A and B feeds into a single UPS feed, rather than A to one and B to the other, just for this row of racks.

The client was understandably unhappy with the electrical contractor responsible for the initial build.
