I'd accept that, when a properly designed and maintained system was working within its predicted operating envelope (including foreeseen failure modes).

Anyone still remember Buncefield, and the analyses that followed?

Where in the lifecycle of "continuous product and service improvement" are Tesla's vehicles, and where are they headed?
