We don't suggest that the cost should come into the pilot's decision at all. They should do what is safest in their judgement, and indeed our participants consistently did this.

In that case you didn't include pilots from airlines where it does come into the pilot's decision as some airlines include those costs into performance reviews and even attach consequences to it. And in case you are interested, those are the same airlines that got fined for consistently skimping on fuel load (insufficient margin for a holding pattern at first alternate airport).

On the other hand, that is already in the article as the most notorious (one could even say infamous) of those flies exclusively with B737s.
