Very good questions indeed.

Broadly speaking, I suggest that anyone who takes the decision to replace a "manual" (human-operated) process with a fully automated process must be responsible for any adverse consequences.

But how the responsibility is allocated - that's a very tricky set of questions.

There should certainly be some kind of precautionary principle: "If in doubt, don't".
