4) Railway and Air Traffic Control Systems
I would not want to test beta software in production when mistakes could mean that planes fall out of the sky. (I have developled ATC system and the rigour of testing, testing and retesting before you get anywhere near production is just part of the culture and it seems to work)
Which leads to
5) Any safety critical system.
