There are also not-fat, ideal-BMI[1] people that develop it in their late 30's. It helps if ones maternal genetic background include a whole bunch of people who developed it early in life, even when living in the days when over-eating was very, very unlikely..

[1] BMI - aka 'a farce'. But the NHS seems to value it (except for the people who really know what they are doing)
