@AC, In your industrial control systems do you include such things as physical key switches that are required to be human activated before any actual switching can take place? We have had to add such switches to almost all of the new control systems that our client has bought because the suppliers couldn't/wouldn't see the necessity for them.
This incident has proved the requirement of our client was eminently justified.
